What should I file to open up my business at home?

I am a mage therapy student and I want to open up a business under a business name (not my name) at my home (considering zone, building restrictions) when I graduate. I will not be selling any products other than my services. I want to promote the business name in my business cards and brochures so that it helps me when I open my clinic later on. How can I save this business name with the city/state? what form should I file or how should I register my business as (e.g. DBA, LLC,)? Thanks in advance.
